Level of Care Assessment

A Level of Care Assessment consists of an evaluation of a client’s life including their drug/alcohol history. This assessment will determine the level of care needed unique to the individual's treatment needs and provide goals for a positive/healthy sober lifestyle. These appointments are two (2) hours long.
Outpatient Program

Outpatient Program (OP) consists of individual therapy and group therapy depending on the individual's unique treatment needs. Treatment hours for OP are anywhere from (0.25 hrs-1x a month individual session to 8.5hours per week--combination of group therapy and individual).
Intensive Outpatient Program

Intensive Outpatient Program (IOP) consists of individual therapy and group therapy depending on the individual's unique treatment needs. Treatment hours for IOP are 9+ hours per week (combination of individual and group therapy).
Individual Therapy/Group Therapy

Individual Therapy
One on one sessions with a counselor include working on the individuals unique recovery plan, building on coping mechanisms to deal with triggers/set backs, providing educational resources, maintaining healthy relationships, removing any negative aspects, and providing relapse prevention. Each client receives one on one therapy throughout all of their program. Each individual therapy session is 1 hour long.
Group Therapy
Group sessions are held with other members in recovery within our programs in a safe and secure environment. This provides an open dialogue of different treatment modalities to help aid in the recovery process. Each group is 2 hours long with a 15 minute break.

Buprenorphine/MAT Services

Facilitated by certified and licensed physicians, buprenorphine/medically assistant treatment services are provided for individuals that are seeking a medication component to their treatment plan under Evolve Addiction Recovery. Separate evaluation and appointments with the physician are required.
Referral Services

Evolve Addiction Recovery has several working relationships with other outside entities so individuals have the best possible care in and out of our program. Referrals range between: inpatient treatment facilities, other drug and alcohol outpatient facilities, food/clothing support, transportation, housing, welfare support, support meetings (AA/NA, Refuge Recovery, SMART Recovery, etc.), primary care, mental health, HIV/STD testing, and more! Each individual gets continuum of care for other outside resources as needed.
